* Indications and Dosage:
– Thyroid hormone replacement.
– The specified dose is to be taken orally once daily before breakfast.
. Adults: Initially, 25 to 50mcg, increased by 25 mcg every 4 to 8 weeks until desired response occurs.
Maintenance dosage is 75 to 200 mcg daily.
. Adults older than 50 years: 12.5 to 50 mcg daily; increase by 12.5 to 25 mcg every 4 to 8 weeks,
depending on response.
. Children older than 12 years (growth and puberty complete): 1.6 to 1.7 mcg/kg daily.
. Children older than 12 years (growth and puberty complete): 2 to 3 mcg/kg daily.
. Children 6 to 12 years: 4 to 5 mcg/kg daily.
. Children 1 to 5 years: 5 to 6 mcg/kg daily.
. Infants 6 months to 1 year: 6 to 8 mcg/kg daily.
. Infants 3 to 6 months: 8 to 10 mcg/kg daily.
. Infants birth to 3 months: 10 to 15 mcg/kg daily.
* Contra-indications:
– Hypersensitivity to any ingredient of the tablets.
– Thyrotoxicosis.
– Acute myocardial infarction.
– Uncorrected adrenal insufficiency.
* Pregnancy: Category A.
Studies have not shown any increased risk of congenital abnormalities. Therefore, the possibility of fetal harm appears remote.
Thyroid hormones cross the placental barrier to some extent as evidenced by levels in cord blood of athyreotic fetuses being
approximately one-third maternal levels. Transfer of thyroid hormone from the mother to the fetus, however, may not be
adequate to prevent in-utero hypothyroidism. “T4 Thyro” should not be discontinued during pregnancy and hypothyroidism
diagnosed during pregnancy should be promptly treated as maternal hypothyroidism may have an adverse effect on fetal
and childhood growth and development. The postpartum “T4 Thyro” dosage should return to the pre-pregnancy dose
immediately after delivery.
* Nursing Mothers:
Although thyroid hormones are excreted only minimally in human milk, caution should be exercised when T4-Thyro
is administered to a nursing woman. However, adequate replacement doses of levothyroxine are generally needed
to maintain normal lactation.
* Warnings and Precautions:
Due to the profound effects of thyroid hormones on energy-requiring metabolic processes, the administration of levothyroxine
to a hypothyroid patient may unmask occult cardiovascular, endocrine or metabolic disease. Levothyroxine should be used with
caution in elderly patients who may be more sensitive to the effects of thyroid hormones.
